By interacting with, submitting information to or signing up for any organized activity offered by Church Name (“Church Name”), you agree and consent to the use, processing, collection and retention of your personal information by Church Name, its staff or appointed committees for the purposes of the church camp activities of Church Name as shall be deemed necessary or desirable.

Liability 
By submitting this registration form, I/ We agree that all the names listed on this form are physically fit and have not been otherwise advised by a qualified medical practitioner. I/ We will not hold the Church and the Camp Committee liable for any accidents or mishaps that may arise from or during the camp. I/ We will abide and comply with all rules laid down by the Camp Committee.
